At the range today shooting a Maadi.  It would fail to eject.  Another time two rounds were trying to load.  I had put the bolt return spring(?) from my Mak onto his Maadi.  Shot it without the dust cover and it ran much better but eventually bolt jammed up and did not charge home.  Barely contacted the next round. Slowly doing action by hand it seems to catch halfway forward.  The upper hand guard has play in it.  The pin that holds it does not make contact.  Extractor spring was much softer compared to my Mak. 

Some pictures of the action and bolt group.  I want to clean out all the rotating parts and contact patches but I do not think that it will fix the issue.  Mags that we used were, us palm, kci (with Wolff springs) and Romanian surplus.  I need suggestions on how to chase this issue.  Cheers!

Other issue is the pin for the folder will slide out with out with some wiggling of the rifle.  Pin is just straight from what I could see.  Did not have a mic with me.

I would say that bolt group has seen better days

The channel in that carrier is extremely rough looking. I will take a look at a few of mine tomorrow if I get a chance but I don’t remember seeing burrs like that on any of them including older WASR rifles.

Is this a USA built kit build?

The FTE can be caused by a few different issues, and you named a couple. A bad ejector spring is one. A loose UHG and gas tube is another. Sounds like to me you are loosing gas pressure. I would check the gas port to make sure it is clear and clean. The other issue where 2 rounds tried to load is a mag issue where the feed lips are spread too wide. Good luck.

That bolt group looks quite worn. If it were mine I would replace the bolt group and update with wolf springs. BUT! I am no expert on troublesome or worn ak. Maybe the guys here can suggest a less expensive alternative first.
