Week follower spring. Replace with a new one.   The first eight rds have enough pressure to feed correctly. The last few the spring cannot push them up strong enough and the bolt comes forward on a misaligned-short reach rd.

My guess is there are a lot more follower springs replaced than needed. 
Mine still have 26 to 30 oz of force to push up the last round. 

Or, we could go on reviving necro threads :)
